Our Vision.
Chronic Care Australia’s vision is to provide healthcare solutions for all Australians suffering from mental and physical chronic conditions. At CCA we are committed to reducing the burden of chronic disease in our metro, remote and regional communities by delivering face to face, virtual and home based exercise medicine programs and services.
Over the past three years we have developed and validated an exercise medicine delivery and prescription system that successfully treats, manages and prevents mental and physical illnesses concurrently. We triage the whole person so that all aspects of health are considered in our treatment plans.
From humble beginnings.
CCA co-founder, Katie Stewart (pictured) was driven by personal experience to find a solution to the current mental and physical chronic disease epidemic in Australia.
“I’ve watched my mother navigate a severe mental illness for 35 years. Over her lifespan, the side effects of prescriptive treatments and the illness itself have impacted the quality of her life significantly, as well as her predicted life expectancy,” she says.
Katie had also seen the high value impact of an effective exercise medicine program on her long-term client and fellow co-founder, Lisa Rowley. Lisa nearly lost her life to encephalitis. Katie worked with Lisa to restore her physical and cognitive function with exercise medicine. This rehabilitation process was the seed that inspired three years of research and resulted in a globally celebrated exercise medicine program called 4 Point Medex that is used clinically throughout Chronic Care Australia’s virtual and face-to-face clinics. This program helps people who have complex chronic conditions live well for longer.
Katie is a passionate and dedicated advocate for combining primary physical and mental health services to treat chronic disease. She sits on the local Advisory Board for the Heart Foundation and has enjoyed supporting and encouraging young exercise physiologists in her role as a sessional lecturer at Curtin University.

Whole person care
At Chronic Care Australia we are committed to delivering patient centred care and services that embrace the whole of the patients’ mental and physical needs. We work with your preferred GP and medical specialists to put together a team of expert allied health care providers to treat, guide and encourage you.
Our Care Coordinators utilise a network of specialist physiotherapists, dieticians, psychologists and occupational therapists that are trained in specialised patient care to ensure all of your allied health care needs are met. throughout your program.
Virtual Care
Our Virtual Care Clinic offers rural and remote patients access to the same high level of care they would receive in the metro areas. Delivering clinically proven exercise medicine programs, the Care Clinic allows us to stay connected to our patients clinical needs, creating a safe community of care and effective treatment delivery.
Our expert allied health therapists don’t just want to treat and protect you from illness and injury. They want you to live well for longer.
How do I access these services?
If you have had one or more of the chronic physical or mental conditions listed below for six months or more, please talk to your GP about a Chronic Disease Management plan and/or a mental health care plan, or simply call one of our friendly staff to find out more about our services.
Medicare and Private Health rebates are available to approved policy and care plan holders at Chronic Care Australia. DVA approved white and gold card holders, NDIS, WAPOL, ACAT and workers compensation insurance claims are also welcome referrals at Chronic Care Australia.
