Real World Evidence.

Value Based Health Care (VBHC) is a model that rewards exercise physiologists for delivering measurable improvements in their clients’ health outcomes - rather than simply providing services - by focusing on prevention, function, and long-term impact. As part of Chronic Care Australia’s commitment to VBHC we apply our 4-Point Medex exercise medicine dispensing system to test, trial and prove exercise medicine efficacy against Australia’s top eight chronic health conditions and patient multi-morbidity.

By capturing Provider-Reported Importance Measures (PRIMS), Patient-Reported Experience Measures (PREMS), and Patient-Reported Outcome Measures (PROMS) during our 12-week Exercise Medicine programs, our aim is to demonstrate real-world impact, support funding conversations, and continuously align care with what matters most to patients. Follow the link to The Take Your Medicine Study to see how this approach drives compliance, adherence, results and loyalty. A breakthrough in the fight against chronic illnesses.
