At Chronic Care Australia, we believe that health extends beyond treating disease. While evidence-based Exercise Medicine is fundamental to improving physical function, strength and longevity, growing research shows that our sense of belonging, purpose and contribution to others is equally important for mental wellbeing and long-term health.

Many people living with chronic conditions experience isolation, loss of confidence and changes to their identity. Recovery is not simply about restoring physical health – it is about helping people reconnect with their community, regain purpose and sense of place.

Whether someone attends an ONERO®, GLAD®, or 12-week Exercise Medicine program, they become part of a community where they are encouraged, supported and valued – not simply treated as a diagnosis.

CCA's Community Programs reflect this philosophy

Each year, through the generosity of donors, sponsors and our community, we provide six months of evidence-based Exercise Medicine to people who may otherwise be unable to access care.

These programs are named in honour of individuals who valued kindness, generosity and community service – recognising that improving health is not only about clinical treatment, but about helping people feel connected, hopeful and supported.

Recipients often tell us that they leave with far more than improved strength or physical function. They leave with renewed confidence, friendships and a belief that they can once again participate fully in life.
